Telehealth Informed Consent Disclosure

Please read this document carefully before using Amazing Meds’ telehealth services. By scheduling a consultation or using our telehealth platform, you acknowledge that you have read, understand, and agree to the terms outlined below.

What is Telehealth?

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. At Amazing Meds, telehealth services are delivered via secure, HIPAA-compliant video conferencing technology.

Services Provided via Telehealth

Amazing Meds provides the following services through telehealth:

  • Clinical evaluations for hormone replacement therapy (HRT), testosterone replacement therapy (TRT), peptide therapy, weight management, sexual health, and hair loss

  • Review and interpretation of laboratory results

  • Prescription of medications, including compounded and retail pharmacy medications

  • Follow-up consultations and ongoing treatment management

  • Behavioral health screening (PHQ-9, GAD-7)

  • Care coordination with pharmacies, labs, and specialists

Benefits of Telehealth

  • Improved access to healthcare without the need for travel

  • Reduced wait times and increased scheduling flexibility

  • Ability to receive care from the comfort of your home

  • Access to specialized providers who may not be available locally

  • Convenient follow-up and ongoing management of chronic conditions

Risks and Limitations of Telehealth

You understand and acknowledge that:

  • Telehealth has limitations compared to in-person visits. Your provider cannot perform a physical examination and must rely on the information you provide, your medical history, and laboratory results.

  • Technical difficulties (internet connection, audio/video quality, equipment malfunction) may occur and could interfere with your consultation. If this happens, your provider may reschedule your appointment or switch to an alternative communication method.

  • In rare cases, information transmitted may not be sufficient to allow for appropriate medical decision-making by the provider.

  • Delays in evaluation and treatment could occur due to technology failures.

  • Although we use encrypted, HIPAA-compliant technology, there is a small risk that electronic communications could be intercepted.

  • Telehealth is not appropriate for all medical conditions. If your provider determines that an in-person evaluation is necessary, you will be referred to an appropriate local provider or facility.

  • Telehealth is NOT a substitute for emergency care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

Your Responsibilities

As a telehealth patient, you agree to:

  • Provide accurate and complete information about your medical history, current symptoms, medications, allergies, and other relevant health information

  • Inform your provider of all other healthcare providers involved in your care

  • Be physically located in a state where Amazing Meds is licensed to provide services at the time of your consultation (see Geographic Limitations below)

  • Ensure you are in a private, quiet location during your consultation

  • Have a reliable internet connection and a device with a camera and microphone

  • Follow your provider’s instructions regarding treatment, medications, laboratory testing, and follow-up appointments

  • Contact your primary care provider or go to an emergency room if your condition worsens or you experience a medical emergency

Geographic Limitations

Amazing Meds provides telehealth services in 45 U.S. states and the District of Columbia.

We do NOT provide services to individuals physically located in: Alabama, Alaska, Arkansas, Louisiana, or Mississippi.

You must be physically located in a state where we are licensed to provide services at the time of each consultation. It is your responsibility to inform us if you relocate or are traveling to a state where we cannot provide services.

Prescribing Policy

  • All prescriptions are issued at the sole discretion of the licensed healthcare provider following a comprehensive medical evaluation and required diagnostic testing

  • A consultation does not guarantee that a prescription will be issued

  • Controlled substances are prescribed only when clinically indicated and in compliance with all applicable federal and state laws, including DEA regulations

  • We do not prescribe controlled substances for off-label uses such as anti-aging, cosmetic, athletic performance, or bodybuilding purposes without documented medical necessity

  • Prescriptions are transmitted electronically to licensed pharmacies

Recording and Documentation

  • Your telehealth consultation will be documented in your electronic health record, just like an in-person visit

  • We do not record video consultations without your explicit consent

  • If recording is requested for quality assurance or other purposes, you will be informed and asked for consent before recording begins

Fees and Payment

  • Consultation fees are for the provider’s time and clinical assessment

  • Fees are non-refundable once the consultation has been completed, regardless of whether a prescription is issued

  • You are responsible for any applicable copayments, coinsurance, or deductible amounts

Your Right to Withdraw Consent

You may withdraw your consent to telehealth services at any time by notifying us in writing. Withdrawal of consent will not affect the legality of any information obtained or actions taken before your withdrawal. If you withdraw consent, you may still receive care through in-person visits with a local provider.
